ABOUT
Improving Diabetes With Population Health Tools
With the focus on improving diabetes outcomes with team-based care, this training will focus on increasing the use of population health platforms to identify patients, communicate outcomes and results with patients, increase patient engagement, and measure performance. A population health platform can also be used to measure and evaluate diabetes care management outcomes effectively. Understanding how to use population health tools to identify diabetic patients through risk stratification and those due for preventative appointments will also be addressed.
